Registration is open to all persons interested in the field of otitis media.

All persons intending to attend the Symposium must complete the online registration form. Please read the registration information below before you complete the online registration form. * To be eligible for the ISOM Member Registration category, you must be a current member of the International Society of Otitis Media. Consider joining the International Society for Otitis Media to access the discounted Member rate. Please allow 24 hours for processing of your membership application prior to registering for the Symposium.

** To be entitled to the advertised registration fee you must have registered and paid by the official closing date for ‘Early’ or ‘Standard’ registration (AEST).

*** Students are required to send a copy of their student identification card to the Symposium Managers via email to otitismedia2017@arinex.com.au or fax to +61 2 9267 5443. Without this copy, the full registration fee will be charged. A Primary Health Care Worker is defined as either a Aboriginal Health Worker/Practitioner, Nursing or Allied Health Professionals, who are based in health care at the community level.

Student and Primary Health Care Workers/Practitioners Registration

A student is defined as a holder of a student identification card from a recognised tertiary or secondary educational institution or international student card.  Students must be studying full time to qualify for the discounted rate.  A Primary Health Care Worker is defined as either a Aboriginal Health Practitioner, Nursing or Allied Health Professionals, who are based in health care at the community level. Student and Primary Health Care Worker registrants are entitled to the following:

    • All Symposium sessions
    • All official documentation
    • Delegate satchel
    • Welcome Reception
    • Delegate catering where provided

 

Registration Information

Registration confirmation
Your completed registration and successful payment will be automatically acknowledged via email with confirmation of your requirements according to your registration form.  Your registration will only be confirmed on full payment of fees due.

Name badge
Each delegate will receive a name badge upon registration on site at the Symposium.  The badge will be your official pass and must be clearly visible at all times during the Symposium to obtain entry to all sessions and to social functions.  The Symposium Managers expect appropriate behaviour from attendees and reserve the right to revoke the badge and privileges of anyone behaving in an inappropriate manner.

Credit Card
Please note all online registrations require immediate payment by credit card. Accepted credit cards are MasterCard, Visa and American Express.  Please note all transactions by credit card will appear on your statement as payment to ‘Conference by Arinex’. If you are unable to pay by credit card, please contact the Symposium Managers via email to otitismedia2017@arinex.com.au.
Payment Options

 

Cancellations must be notified in writing to the Symposium Managers via email to otitismedia2017@arinex.com.au.

Registration Fees:
Cancellations before 17 March 2017 will incur an 150.00 Australian dollar cancellation fee.
Cancellations before or on 28 April 2017 will receive a 50% refund of fees paid.
Cancellations after 28 April 2017 there will be no refund.

Accommodation Fees:
A minimum one night’s deposit must be paid or credit card details given at time of booking to guarantee reservation. Deposit is non-refundable at 28 April 2017.

Social Tickets:
Payments are non-refundable at 28 April 2017.

Cancellation and Refund Policy 

 

Registration fees do not include insurance of any kind. It is strongly recommended that at the time you register for the Symposium and book your travel you take out an insurance policy of your choice. The policy should include loss of fees/deposit through cancellation of your participation in the conference, or through cancellation of the Symposium, loss of international/domestic air fares through cancellation for any reason, loss of tour monies through cancellation for any reason including airline or related services strikes within Australia, failure to utilise tours or pre booked arrangements due to airline delay, Force Majeure or any other reason, medical expenses (including sickness and accident cover), loss or damage to personal property, additional expenses and repatriation should travel arrangements have to be altered. The Symposium Managers cannot take any responsibility for any participant failing to arrange their own insurance. This insurance is to be purchased in your country of origin.

This information is correct at the time of publishing.  The organisers reserve the right to delete, modify or alter items or to delete, modify or alter any aspect of the Symposium timetabling and delivery at their sole discretion and without notice.  Neither the host organisation(s) nor the Symposium Managers will accept any liability for any loss or inconvenience caused to any party consequent to such changes.

Recordings / Copyright
The Symposium Managers reserve the right to use any recordings whether audio or video and/or any photographs taken at any stage during the Symposium for promotional, advertising or reporting of the Symposium and/or its content at their sole discretion.
